Accepting this preliminary agreement does not conflict with any other IPA, ACO, or health plan relationships that you may currently have. Additionally, there are no fees, dues, or risks contracts associated with accepting this agreement. At Ovation Health we work to ensure our participating physicians receive reimbursement rates that are competitive in the market with no downside risk for the provider.
By signing below, you and/or your practice acknowledge that you have opted into agreement with the terms specified in the “Provider Rate Chart for PPC TPA Administered Ovation Health Plan with with USA Primary Care”.
Physicians under this agreement are subject to credentialing through PPC TPA and are required to adhere to standards set forth in the Ovation Health Provider Manual. Click on the button below to download the manual.
