Additionally, there are no fees, dues, or risks contracts associated with accepting this agreement. At Ovation Health we work to ensure our participating physicians receive reimbursement rates that are competitive in the market with no downside risk for the provider.

By signing below, you and/or your practice acknowledge that you have opted into agreement with the terms specified in the “Provider Rate Chart for PPC TPA Administered Ovation Health Plan with with USA Primary Care”.
